Thank You for choosing Prescott Children's Academy. Here you can print Forms to bring in to enroll your child. Remeber we need a copy of your child's immunization records. The medical form you print must be filled out by your child's physician, nurse, or medical asst. There will be additional papers we will give you at the center.
